There are several ways to request a crash report including:
To request an accident report by email:
Drivers must submit a completed request form (PDF). You will need to meet an appropriate authorization under Section III on the request form to receive an unredacted accident report. If the accident was reportable to the Wisconsin Department of Transportation (DOT), we will provide you with the DOT document number (PDF) for requesting directly from their agency; if the accident was not reportable to the state, we will provide you with an accident summary via email. There is no charge for this service.
Insurance companies and attorneys should include the report number (if known), the date of the accident and the name of the insured/client. If the accident was reportable to the Wisconsin Department of Transportation, we will provide you with the DOT document number (PDF) for requesting directly from their agency; if the accident was not reportable to the state, we will provide you with an accident summary via email. There is no charge for this service.
If an insurance company or attorney wishes to request the supplemental report for an accident, requests must be submitted in writing on letterhead together with a completed Record Request Form (PDF); requests should include the name of their insured or client. No one is allowed access to unredacted reports in order to seek future clients.
